The son of R. Sinai of Kaushitz and Zemigrad, who was the son of R. Baruch of Gerlitz, and the son in turn of the Divrei Chaim. R. Ya'akov Halberstam married the daughter of R. Shalom Shatz of London. He was appointed Av Beth Din in Chakova and in 1942 came to Jerusalem, where he established Yeshivas Divrei Chaim-Chakova. He spent time in New York and was active in the Va'ad Hatzala Organization seeking to rescue the Jews of nazi-occupied Europe. See Encyc. HeChassidus, col. 234.
